Required Documents

  1. Two colour photos
  2. Pan card
  3. Aadhar card
  4. Light bill of your office/ premises
  5. Registered Office Address proof:-
    –Self-owned property – Copy of electricity bill, landline bill, water bill, municipal khata copy, property tax receipt
    –Rented property – Rent agreement and No objection certificate (NOC) from the owner of the rented property on Stamp of Rs. 500/-
  1. NOC and copy of License from the manufacturer
  2. Copy of certificate obtained under Coop Act 1861/Multi state Coop Act 2002
  3. Food safety management system plan or certificate

Additional Documents Require

01. In Case of State License-

  1. Plan of the processing unit showing the dimensions and operation-wise area allocation
  2. List of Directors/ Partners/ Proprietor with address, contact details, and photo ID
  3. Name and list of equipment and machinery used with the number and installed capacity
  4. List of food category to be manufactured
  5. Authority letter from manufacturer nominated a responsible person name and address
  6. Analysis report of water to be used in the process to confirm the portability
  7. Proof of possession of premises
  8. Partnership deed/ affidavit of proprietorship

02. In Case of Central License-

  1. Form B duly completed and signed
  2. Source of raw material
  3. Ministry of Commerce Certificate for 100% EOU
  4. NOC/PA document issued by FSSAI
  5. IE code document issued by DGFT
  6. Form IX
  7. Certificate from Ministry of Tourism
  8. Proof of possession of premises
  9. Partnership Deed/ Affidavit of Proprietorship
  10. NOC and copy of License from the manufacturer
  11. Food safety management system plan or certificate
  12. NOC from the municipality or local body
  13. Supporting document for proof of turnover and transportation
  14. Declaration form
